Pelviscopic Management of Ectopic Pregnancy





Abstract
One hundred thirty ectopic pregnancies were treated with a pelviscopic procedure from August, 1989 to May. 1993. Surgical techniques included salpingestomy, sapingostomy. salpingotomy, and so on. Sapingectomy using bipolar cauterv and laser was performed most frequently. Linear salpingostomy using the contact Nd : YAG laser was our surgical method of choice, in patients with unruptured tubal. pregnancies, who desired further fertility. The. mean operating time was 27 minutes(range. 15 to 90). The average length of hospitalization was 4.2 days(range, 2 to 8 days). One case of incomplete removal of ectopic conception. product during salpingostomy was a major complication which was managed by systemic methotrexate therapy. Twenty three of 32 patients treated with conservative surgery actively try to conceive, 15(65.2%) conceived, and one (4.3 %) was a repeat ectopic pregnancy. The intrauterine pregnancy rate was 60.8%. This study indicates that the majority of ectopic pregnancy can be treated by pelviscopic procedure successfully and effectively even though in the presence of rupture, with low morbidity, short hospitalization, and early return to full activity.
